การบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่นเพื่อดูแลผู้สูงอายุในภาวะพึ่งพิง: กรณีศึกษาเทศบาลตำบลร่มเมือง จังหวัดพัทลุง

บทคัดย่อ
การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่นและประเมินประสิทธิผลของเทศบาลตำบลร่มเมือง จังหวัดพัทลุง ในการดูแลผู้สูงอายุในภาวะพึ่งพิง โดยใช้กรอบแนวคิดการบริหารจัดการภาครัฐแนวใหม่ (New Public Management: NPM) และแนวคิดการดูแลระยะยาว (Long-Term Care: LTC) เป็นการวิจัยเชิงคุณภาพ เก็บรวบรวมข้อมูลจากผู้ให้ข้อมูลสำคัญจำนวน 15 คน ประกอบด้วยผู้บริหารและเจ้าหน้าที่ 6 คน และอาสาสมัครที่เกี่ยวข้อง 9 คน ระหว่างเดือนมิถุนายน-ตุลาคม พ.ศ. 2568 ผ่านการสัมภาษณ์เชิงลึกด้วยแนวคำถามที่ผ่านการตรวจสอบความเหมาะสมจากผู้ทรงคุณวุฒิ จากนั้นวิเคราะห์ข้อมูลด้วยการวิเคราะห์เชิงเนื้อหา (Content Analysis) ตามกรอบแนวคิด NPM และตรวจสอบความน่าเชื่อถือด้วยวิธีสามเส้า (Triangulation) ใน 4 มิติ ได้แก่ ข้อมูล (Data Triangulation) วิธีการ (Method Triangulation) ผู้วิจัย (Investigator Triangulation) และทฤษฎี (Theory Triangulation) ผลการศึกษาพบว่า เทศบาลมีการบริหารแบบมีส่วนร่วม โดยประเมินภาวะพึ่งพิงด้วยเครื่องมือ Activities of Daily Living (ADL) จัดทำแผนดูแลรายบุคคล (Care Plan) และจัดสรรงบประมาณอย่างเป็นระบบและโปร่งใส ส่งผลให้ผู้สูงอายุที่มีภาวะพึ่งพิงทั้ง 44 คน เข้าถึงบริการได้ครบถ้วน มีคุณภาพชีวิตดีขึ้น ภาระผู้ดูแลลดลง และเกิดเครือข่ายการดูแลที่ยั่งยืน อย่างไรก็ตาม ยังพบข้อจำกัดด้านงบประมาณ บุคลากรเฉพาะทาง และระบบข้อมูลที่ไม่บูรณาการ ดังนั้น จึงควรพัฒนาศักยภาพบุคลากร เพิ่มความยืดหยุ่นในการบริหารจัดการกองทุน และพัฒนาระบบฐานข้อมูลกลางเพื่อให้ระบบการดูแลระยะยาวมีความยั่งยืนและสามารถขยายผลได้
